Housing Support-LTH Contract Application
Standard application and screening process for qualified service providers (vendors) interested in serving the Housing Support - LTH population.
Housing Support (formally GRH) is a State-funded program that provides an income supplement each month to pay for rent and food. All of those supported by the program are at risk of institutional placement or homelessness.
During the 2015 legislative session, the Minnesota Legislature enacted policy changes to the Housing Support program. One of those changes was the individual eligibility for supportive services. Qualifying persons no longer have to exit from a qualified homeless shelter in order to receive supplemental services. Instead, people must meet Minnesota’s long-term homeless (LTH) definition (in addition to being eligible for the program), and be referred by their local homeless Coordinated Entry system for supportive housing. This program is often called Housing Support-LTH or scattered site Rate 2 Housing Support.
