Eligibility

Criteria


Eligibility for Adult Mental Health Case Management Services is based on a Mental Health Professional’s determination that the individual needs the service and meets the State’s requirements as having a serious and persistent mental illness (SPMI).

The individual must have a mental illness and meet at least one of the following criteria:
  • Two or more episodes of inpatient psychiatric care in the past two years
  • Continuous psychiatric hospitalization or residential treatment of more than six months in the past year
  • Treatment by a crisis team two or more times in the past two years
  • Diagnosis of Schizophrenia, Bipolar Disorder, Major Depression or Borderline Personality Disorder; significant impairment in functioning ; and opinion from a Mental Health Professional that the individual is likely to have future episodes requiring inpatient or residential treatment (of the frequency stated above) unless case management services are offered
  • Commitment or stay of commitment as mentally ill in the past three years
  • Eligible for services but time period expired OR eligible as a child for Children’s Mental Health Case Management & a written opinion by a Mental Health Professional (as above)
