Case Management Offers assistance
to seriously and persistently mentally ill adults in obtaining necessary
mental health, social services, financial assistance, medical, legal, vocational,
housing, and other services designed to facilitate their adaptive community
adjustment.
Crisis Case Management Officers
services to adults experiencing mental health crisis, including hospital
or community visits, referral to appropriate mental health services and/or
financial assistance programs, and other practical assistance in order to
prevent unnecessary hospitalization.
Mental Health Center
Counseling according to individual needs, such as personal, parent-child
relationships, problems with aging, child and adolescent therapy, hospital
commitment and aftercare, routine and court-ordered psychological evaluation,
help clients deal more effectively with everyday problems, improve communication,
and clarify personal goals to help prevent mental distress. Offer group
sessions and special support groups. Third-party or personal payment on
sliding-fee scale.
Custody Mediation Services
Discuss with parents the decision-making issues of custody and visitation
which result in a contractual agreement by the parties sent to the Court.
Only court-ordered.
Community Support Program/Adult Rehabilitative
Mental Health Services Housing Assistance. Individuals will
be assisted in accessing and maintaining appropriate community housing.
The housing coordinator will teach individuals independent living skills,
monitor community adjustment, and provide crisis assistance.
Nursing Registered nurses
provide a variety of psychiatric support services including medication monitoring,
home visits, coordinating and accompanying persons to medical appointments,
mediation education and support groups.
Psychiatric Day Treatment
Provides group experiences for adults needing support and socialization
in order to better adapt to work, family, and life in the community - an
alternative to hospitalization or as a transition from hospital to home
as well as providing help for those experiencing a crisis in their lives.
Independent living skills programming.
Psychiatry Medication
monitoring, psychiatric evaluation, other medical interventions, and clinical
consultation. Tardive dyskinesia screening.
Second Step Provides
a safe and secure environment for adults with a serious and peristent mental
illness living in the community. Programming focuses on life and leisure
skills development, educational programs, and social-recreational activities
designed to facilitate members integration into the community. For more
